When a child scores in the “concerning” area on the Ages and Stages Questionnaire (ASQ) an immediate referral to appropriate community agency is necessary.
Providers from across the state of Colorado have praised the ASQ, saying that it eliminates guesswork and enables families to be connected to services they need and are entitled to at the earliest opportunity.
ABCD is available to assist you every step of the way. Please contact the ABCD team for assistance with the referral process.
As with medical referrals, it is important to track developmental referrals, in order to know whether you have received the appropriate feedback and to be able to follow-up with your patient’s family. Click here to download the ABCD Referral Tracking Form (.xls).
