Early Intervention Colorado – for children Ages Birth through 2 Years

From birth to a child’s 3rd birthday, referrals are made to Early Intervention Colorado.

• Step 1: Identify the child’s local community center board by entering their County, State and Zip Code.
• Step 2: Download the Referral Form in English or Spanish
• Step 3: Complete the form and fax it along with the child’s most recent ASQ results to the contact provided
• Step 4: A Referral Status Update Form will be faxed back to you. This will update you on the status of the child, i.e. date of evaluation, whether or not they were found eligible, and services they will be receiving. To view an example, click here. (PDF)

Early Intervention is sometimes referred to as 'Part C' as it is listed under the Individuals with Disablities Education Act (IDEA) of 2004.
